Patient Care Management
No one organization owns our regional health care delivery system. It is a
patchwork of public, nonprofit and for-profit organizations and individual
practitioners. This describes most health care markets and typifies the
"American way." Still, this can be overwhelmingly complicated, fragmented and
inconsistent from a patient's perspective.
Collaborative system development is a way to organize care among multiple
owners and keep health care decisions local--preferably between a patient and
doctor. Patient care management initiatives will link cental western Washington
health care services to create a coordinated regional system that is easy to
access and navigate as a patient transitions from one level of care to another
or among facilities. The focus of our activities will be on projects, such as
standardizing the patient transfer process and electronic transfer of patient
records.
Over the next two years we will continue to:
- Collaborate with public health on local service coordination
- Make it easier for patients to access services and refer to providers
within the region
- Inventory oncology services and recommend coordination strategies
- Identify and facilitate training on rapid response to acute stroke
- Launch a care coordination functional team to design an ideal model to
demonstrate in our region.
