ConneXions is a . . .
 Community-driven project of social service,
government, faith-based and health care
organizations working together to connect individuals and
families in need to services. Our goal is to reduce system
fragmentation and barriers by simplifying and
coordinating access to local resources in Grays
Harbor, Lewis, Mason, Pacific and Thurston counties.
We will accomplish this through a community-owned
information system that includes a screening and
assessment process, a comprehensive resource
database, and a coordinated referral system. ConneXions
will help local organizations serve their clients
more effectively and efficiently through the use of
new technology, while at the same time enabling
better stewardship of resources.
